PATCH Programs provides residential habilitation and personal care services for children and adults. Our services are designed to support individuals with intellectual disabilities, mental health conditions, and behavioral challenges while promoting independence and community inclusion.

Girls Residential Homes (14–20 years of age)

Our girls’ residential homes provide a safe and structured environment for ages 14–20. We support individuals with mental health conditions, behavioral challenges, and developmental needs.

Each resident receives personalized care focused on emotional well-being, independence, and daily life skills. Our goal is to help them grow with confidence and successfully integrate into their communities.

Serving Girls with Conduct Disorder, Mental Health & Developmental Needs

We provide specialized care for girls diagnosed with conduct disorder, mental health conditions, intellectual and developmental disabilities, and autism. Our programs focus on improving behavior, emotional regulation, communication, and social skills through structured support and individualized care plans.

Boys Residential Homes (8–12 years of age)

Our boys’ residential programs are designed for ages 8–12 who require structured support for behavioral and mental health challenges.

We help children develop discipline, confidence, and essential life skills while providing a safe and supportive environment tailored to their individual needs.

Habilitation Services for Individuals with Intellectual Disabilities

We offer residential habilitation services for individuals with intellectual and developmental disabilities.

Our programs focus on building independence, improving daily living skills, and encouraging active participation in the community through structured and supportive care.

Residential Housing for Adults with HIV

We provide safe, inclusive, and supportive housing for adults living with HIV and other specialized care needs.

Our services promote dignity, stability, and independence while ensuring access to proper care, support systems, and a positive living environment.
